Consultative Lab Visit Request
EHS Research Health & Safety offers non-punitive safety and health assistance to all U-M research laboratories. Simply complete this form to request an EHS consultative visit to your Lab. Our experienced professionals will help you identify hazards and answer any questions you have. All information is provided in a non-punitive manner with no deficiency citations. Together, we can tour your lab to suggest ways you can create a safer and healthier environment for your workers. A deferral from programmed lab inspections is provided during the time labs are working proactively with EHS to survey and correct hazards.

Code Inspection Order

Request form for all non-AEC projects requiring mechanical, electrical, plumbing, and/or certificate of occupancy inspections.

All of the following fields must be completed by the requesting department.

NOTE: Work Order is for reference only. Valid Short Code is required for inspection.
The inspection fee will be $75/hour for time and material projects or 1% of construction cost whichever is greater.

Hydrant Flow Test Request
Environment, Health & Safety’s Fire Safety Services group will assist the project team with identifying water supply and hydrant coverage based on the size of a building and the construction type.

Note that hydrant testing has weather and ground temperature limitations. Generally, between November and April hydrants are drained for cold weather. At any time when temperatures can result in freezing conditions on roads and sidewalks, testing may be postponed.

Reason for the flow test *
If applicable, attach area map or drawing indicating which hydrants to be tested. Generally, one hydrant will be used for measuring flow and a second hydrant measuring static and residual pressure.

Food Truck Application

While the Food Truck Application Form only needs to be submitted and approved once every six months (January-June or July-December), the Notice of Intent to Operate form must be submitted every time a STFU or Mobile Food Establishment operates at an event on campus. Asbestos and Lead Survey

The following form is to be used when requesting air monitoring services during asbestos abatement projects (regulatory requirement for all asbestos abatement projects), or when requesting a building materials survey prior to the beginning of a renovation or demolition project.

For air monitoring, requests should be made at least 48 hours in advance if possible. For building materials surveys, requests should be made 10 or more business days (2 weeks +) prior to the date results are needed. For emergency air monitoring during damaged asbestos cleanup, or for smaller renovation projects requiring faster survey result turnaround times, please call (734) 647-1143.
